We are pleased that you are enrolling in our schools. Please review the information below learn where to register and what documents are required for an efficient registration process.

Student registering for Kindergarten prior to the school year should follow instructions on the Kindergarten Registration web page.
 

Students are enrolled in the Bowling Green School District under strict guidelines established by Ohio Law - Ohio Revised Code, especially the Missing Children's Act. New students to the District are required to present the following documents upon registering to attend Bowling Green Schools:

• Child's birth certificate - original certificate with embossed seal (we will copy and return to you)
• Child's Social Security Card - original card (we will copy and return to you)
• Proof of required immunizations - see requirements
• Proof of residency - current utility bill, rent receipt, phone bill, etc. containing custodial parent/guardian name, address, and current date
• Proof of custody (if applicable) - original court document stating custody guidelines
• Parent Photo ID - driver's license, student ID, etc.
• Current IEP documents - for special needs students

 Parents may also complete in advance and submit during registration:

• Registration Checklist(pdf) - please bring with you
• Permission to Release Student Information Sheet (pdf) - due at registration
• Student Registraton Form(pdf) - due at registration
• Emergency Medical Form (pdf) - due at registration
• Bus Transportation Form (pdf) - due at registration
• Release of Records Form for High School Students (pdf) - due at registration
• Release of Records Form for Middle School Students (pdf) - due at registration
• Release of Records Form for Elementary School Students(pdf) - due at registration
• Medication Form (pdf) - for students requiring medication/inhalers (prescription or non-prescription) during the school day.
